Privacy Requests & Complaints

What you can ask for
- Access — confirm whether we collect or share your consumer health data or other personal information, and receive a copy, including the third parties we shared it with.
- Correct — fix inaccurate information we hold about you.
- Delete — have your consumer health data deleted, including from archives and backups, with notice to the processors and third parties who received it.
- Withdraw consent — to collection or sharing of consumer health data, to email marketing, or to SMS messages.
- Copy of your medical record — HIPAA-protected records are requested under our Notice of Privacy Practices and require a signed authorization.
- Appeal — challenge a denial of any request.
How to submit a request
Email privacy@botanica-beauty.com with the subject line "Privacy Request", or mail or call the Privacy Officer at the address above. Tell us: your name, a way to reach you, whether you are or have been a patient, and which right you are exercising. An authorized agent may submit on your behalf with written permission that we can verify.
How we verify your identity
Before acting on a request we confirm that you are who you say you are, using information we already hold — for example, matching the email address or phone number in our records, or, where the request is sensitive, asking for government-issued identification reviewed and then discarded. We collect only what is needed to verify you and never use verification information for marketing. If we cannot verify you, we will explain why and what would allow us to proceed.
Timing
We acknowledge requests promptly and respond within 45 days. We may extend once by an additional 45 days when reasonably necessary, and will tell you before we do. The first two requests in any twelve-month period are free.
Appeals
If we decline your request, our response will explain why and how to appeal. Send appeals to privacy@botanica-beauty.com with "Privacy Appeal" in the subject line; we respond within 45 days. If we deny your appeal, you may complain to the Washington State Attorney General at atg.wa.gov/file-complaint.
Complaints
You may file a privacy complaint with our Privacy Officer at any time. For HIPAA-protected health information you may also complain to the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/ocr/complaints. For concerns about a licensed provider, contact the Washington State Department of Health at doh.wa.gov. We will never retaliate against you for filing a complaint or exercising a privacy right.
